Scanner Audit Form on the Website

We are finally going to start tracking properly who has what scanner and which ones are going in for repair. There is now a new form on the website and the button is a light green to make it stand out and easy to see as seen below.
This button will take you to the google form where we can log what scanner you have or log the scanner that you are sending in to repair.
Here is when you need to use this form:
This first time we just need you to fill the form in with the current 13 digit number that identifies your scanner.
If you ever send your scanner in for repair I need you to then fill in the form again and answer what is the issue with the scanner.
Once you receive your replacement scanner to use while your scanner is in for repair I need you to fill this in.
When you receive your scanner back from repair I need you to fill this form in again to let me know you have your scanner back.
If, for what ever reason you swap scanners with another person I need this form filled in by both people.
To find the 13 digit number that identifies the scanner you need to press the circle button at the bottom of your scanners screen, seen in the below picture in the pink circled area. This will bring the screen up that you see in the picture below. The number is the 13 digit number at the top of the screen shown in the picture below in the light blue circle with the arrow pointing to it. Finally, to return to the part of the scanner we all use you will need to press the "Scan it PUD" symbol in the bottom left corner which can be seen in the picture below in the green circled area.
